The Business Info section of the data collection form is a mandatory section.
It allows you to select the Form type and enter data about your business.
However many customers may preventing you from changing it by locking the Form type
Class A - Query List Only should be used to declare whether or not your product is compliant (or contains substances below threshold) or not compliant (or contains substances above threshold) or is compliant by exemption to a preset list of environmental regulations.
Class C - Material Summary should be used to provide details of substances of concern that are over reportable thresholds.
Class D - Full Material Disclosure also known as FMD, should be used to declare all chemicals and materials used in your product.
Some sections may have been auto-populated by your customer. To change this data, simply overwrite.
All mandatory fields are in red. You will not be able to validate and lock the form until there is valid data in each mandatory field.
Click the Copy details button to copy the Contact Name, Contact Title, Contact Phone, and Contact Email to the Supplier Authorized Representative fields if they are the same person at your organization.
Click the Validate and lock section button when done completing the mandatory fields. Correct any errors as applicable.
